A concrete products manufacturer has been fined £100,000 for failing to properly risk assess the stability of work equipment after a 1.5 tonne steel beam fell from a crane and fatally crushed a worker.
An investigation found the company had failed to produce a suitable risk assessment to identify the measures it should have taken to prevent this happening.
The company had to pay a further fine of £42,471 in costs after it admitted breaching section 2(1) of the Health and Safety at Work Act.
